Any person who, with the intent to defraud or deceive, submits an application, containing any false, incomplete or misleading information, or helps in any manner to commit a fraud may be subject to civil penalties and criminal prosecution for insurance fraud.
Notice Regarding the Privacy Act
A credit report or other investigative report about you may be requested in connection with an application for insurance. Any information which we have or may obtain about you or other individuals listed as policyholders on your policy will be treated confidentially. You have the right to see personal information collected about you and you have the right to correct any information which may be wrong. Upon your request the address of the reporting agency will be supplied so that you can request this information.
